    Introduction
    Safe-Connect ID is a voluntary, opt-in public safety data registration platform developed and managed by Mental Verification Technologies. This platform is designed to assist authorized law enforcement officers and emergency responders in identifying and interacting more safely with individuals who have disabilities, neurodevelopmental conditions, or mental health challenges. The platform allows individuals, or their legal representatives, to submit relevant non-emergency personal, demographic, and health-related information.

By using this platform, the user (referred to as the "Registrant") agrees to the terms set forth in this Disclaimer and Terms of Use.

    Voluntary Participation
    Participation in Safe-Connect ID is entirely voluntary. Users may revoke their participation or request updates to submitted information at any time by submitting a written request to Mental Verification Technologies. Participation does not constitute legal registration with any government agency and does not guarantee any specific response or outcome by law enforcement or emergency personnel.

    Scope and Limitations
    The Safe-Connect ID platform is designed to be a national public safety resource; however, it is currently in the phased implementation stage. While law enforcement and emergency response agencies are actively enrolling as authorized users, the platform is not yet universally adopted or accessible by all public safety entities across the United States.

The Registrant acknowledges and agrees that enrollment by public safety agencies is ongoing and that full adoption and integration across jurisdictions will occur incrementally over time. Accordingly, the submission of data to the Safe-Connect ID platform does not create any legal duty or obligation on the part of non-participating agencies to access, review, or act upon such information.

Mental Verification Technologies expressly disclaims any liability or responsibility for adverse outcomes, delays, or failures to act that may result from the unavailability, inaccessibility, or non-utilization of data by agencies that have not yet enrolled in or integrated with the platform.

Participation in the platform should not be interpreted as a guarantee of service, outcome, or data access by any specific responding agency, particularly in jurisdictions where onboarding is still in progress.

    Legal Authority to Submit Information
    By submitting data through the Safe-Connect ID platform, the Registrant affirms, under penalty of perjury, that they are either:

    The individual subject to the data and are of legal age and sound mind to provide such consent; or

    The legal parent, guardian, or duly authorized representative of the individual subject to the data, possessing full legal authority under applicable state or federal law, including powers of attorney, conservatorships, or guardianship rights.

Submission without proper legal authority constitutes a violation of this agreement and may subject the individual to legal consequences.

    Accuracy and Truthfulness of Information
    The Registrant represents and warrants that all information submitted is true, complete, and accurate to the best of their knowledge. Any knowing or willful falsification, misrepresentation, or fraudulent entry of data may result in civil liability and/or criminal prosecution under state and federal laws. This includes potential charges related to identity theft, fraud, obstruction, or false statements to law enforcement.

The Company reserves the right to report suspected abuse or misuse of the platform to the appropriate authorities.

    Data Use and Privacy
    Information submitted may include personally identifiable information (PII) and protected health information (PHI). All data is handled in accordance with applicable privacy and security standards, including the Health Insurance Portability and Accountability Act (HIPAA), to the extent such regulations apply.

The Company uses administrative, physical, and technical safeguards consistent with industry standards to protect your information. Information will be disclosed only to authorized public safety personnel for the sole purpose of enhancing response effectiveness during calls for service.

    Disclaimer of Warranties
    Safe-Connect ID is provided “as is” without warranties of any kind, express or implied. The Company makes no guarantees regarding the completeness, accuracy, timeliness, or effectiveness of the platform or data accessed by authorized personnel. Users acknowledge that platform performance may be subject to limitations beyond the control of the Company.

    Indemnification
    The Registrant agrees to indemnify, defend, and hold harmless Mental Verification Technologies, its officers, directors, employees, and affiliates from and against any and all claims, demands, losses, liabilities, costs, and expenses (including reasonable attorney’s fees) arising from:

    The Registrant’s use or misuse of the platform;

    Submission of inaccurate, unauthorized, or unlawful information; or

    Any breach of this agreement.

9. Governing Law and Jurisdiction
This agreement shall be governed by and construed in accordance with the laws of all 50 States of the United States, without regard to its conflict of law principles. Any legal action or proceeding arising from or related to this agreement shall be brought exclusively in the courts located in the parties jurisdiction hereby submit to the jurisdiction of those courts.

 Before initiating any litigation, the parties agree to first attempt to resolve any dispute, controversy, or claim arising out of or relating to this agreement through good faith mediation. The mediation shall be conducted in Dallas, Texas, under the rules of a mutually agreed-upon mediation service or mediator. If the parties cannot agree on a mediator within ten (10) business days of a written request for mediation, either party may request that a mediator be appointed by a court of competent jurisdiction in Dallas, Texas.

Each party shall bear its own costs for mediation, and the parties shall split the costs of the mediator equally unless otherwise agreed. If the dispute is not resolved through mediation, either party may pursue legal remedies in accordance with this agreement.


10. Consent and Acknowledgment
By checking the acknowledgment box or signing this document (for print use), the Registrant confirms the following:

    They have read and understood this Disclaimer and Terms of Use in full.

    They affirm they have the legal authority to submit the information.

    They accept all terms, limitations, and legal responsibilities associated with their use of the Safe-Connect ID platform.
